Utah Physicians and Surgeons Licensing Board licenses physicians in Utah, for both MD and DO applicants. For an Utah hire, the initial application fee is $200, licenses renew on a biennial cycle and renewal carries a CME requirement (40 hours per two-year cycle, at least 34 hours ACCME Category 1; controlled-substance prescribers must complete at least 3.5 hours of Division-approved continuing education). Utah's compact status is participating non-SPL.
